Online CRM Form Setup

The Online CRM Form provides clubs with the ability to capture new prospects by way of a form on their ClubHouse Online website. When the form is submitted, a new prospect will be created in JAM/Encore according to the settings configured in the application. Basic demographic information (First Name, Last Name, Email Address, Street Address, City, State, Country) will be captured and mapped to the appropriate fields in Encore/JAM, while clubs can add custom questions which will be added to a Note associated with the Prospect.

Creating a CRM Integrated Form

Within Club Central, users will be able to create/edit CRM forms by using the BizForms module. Instead of a single option to create a New Form, click on the option ‘New CRM Form’.


Upon opting to create a New CRM Form, users will be prompted to supply a Display Name for the Report.

Since this is a CRM form, the default fields are added to the report and users can then choose to add more Custom Fields and/or modify the layout of the report, just like any other BizForm.

Automatically added fields cannot be removed by way of the Form Builder, however, they can be removed by way of the Fields tab. Removing any of the automatically added fields, especially the mandatory fields, is not recommended. Removing any of the automatically added fields will result in less data being collected and synced to CRM, while removing the mandatory fields will prevent the system from evaluating duplicate entries.

Once the form is completed, clubs can insert this form on any content page which allows for the insertion of a BizForm.
